A daycare sign-in sheet is a form that parents fill out when dropping their children off at daycare, preschool, afterschool, or other child care programs. They sign it again when they pick their children up. This provides an official record of when the child was in the care program.
Childcare providers must follow the policies and procedures of their licensing agency to ensure child safety. As part of these regulations, many states require the use of daycare sign-in sheets to help track attendance. If you aren’t sure about the laws in your state, follow this link to review them. Some states, like Wisconsin, even require family childcare centers that accept a childcare subsidy to use a specific form. But, most centers are expected to create their own attendance log.
Even if such records are not mandatory for your childcare licensing, there are still benefits to using attendance sheets. They help the staff know exactly which children are on-site. This colorful sign-in sheet is dated and designed to be printed daily. As parents drop their children off in the morning, they fill in the form. Then the next parent signs in below them.
With this worksheet, teachers can see at a glance which children were in their care on a particular day.
However, it does not have a spot for a parent signature, so if your regulations require that, this version may not be a good fit, unless you change one of the existing columns to “signature.”
Daily In Out Month Portrait Daycare Sign-In Sheet
Some daycare centers prefer to keep individual attendance records. This sample form does that. Each month, you print out a copy of the attendance tracker for each child in your care.
As the parents arrive, they locate their child’s form and sign in. When they leave, they sign them out.
With portrait orientation, this free printable worksheet easily fits into a three-ring binder or on a clipboard.

You can customize this sign-in sheet to match your daycare’s branding.
Click on the name “Morning Glory Daycare” to enter in the name of your childcare center. If you need a space for a parent or guardian signature, you can change the Notes column to read Signature. If you want to collect some other information (allergies, medication administration, etc.) you can change this column to fit your needs.

The column “Notes” can be changed to include other information you want to collect – for example, emergency contact, special meal information, etc.
When you finish customizing your childcare center form, it’s ready for use. Just make sure you change the date before printing!
You can print out a new form each day, or you can print them in advance for the whole week. But if you do this, remember to verify that you use the correct date!

You will need to print out a copy of this attendance tracker for each child in your care. You can either type in the child’s name, the month, and the year, or you can manually write it in after you print them.
